In order to apply for a housing accommodation, such as a medical single, you will first need to register with Disability Services (DSC) as detailed in the directions below, and any request with regard to your housing will be reviewed by a committee.
For you to register with our office, we at the Disability Services (DSC) need you, the student, to submit a "Getting to Know you Form" and documentation on your disability.
You can find the "Getting to Know you Form" and the required documentation on our website. (Links below) You can upload your documentation at the end of your Getting to Know You Form before submitting it. If you are having trouble uploading it you can scan and email me a PDF and I can upload it for you. You can download a free scanning app like Microsoft Lens.
Once the DSC receives your documentation, one of our counselors will reach out to you to schedule an appointment. Reasonable Accommodations will be discussed at that time.
